Tattoo Consent & Release

In consideration of receiving a tattoo from Heritech Social Club including its artists, associates, apprentices, agents, or employees, I agree to the following:

I have been fully informed of the inherent risks associated with getting a tattoo. These risks, known and unknown, can include infection, scarring, allergic reactions, pigment reactions, and complications during healing.

I am not under the influence of alcohol or drugs, and I am voluntarily submitting to be tattooed without duress or coercion.

I assume responsibility for taking care of my new tattoo as instructed by my artist and according to the aftercare instructions I have been given.

I confirm that I have disclosed any diabetes, epilepsy, hemophilia, heart condition, blood thinning medication, skin condition, pregnancy or nursing status, blood-borne disease, or other condition that may affect the tattoo procedure or healing.

I understand that Heritech Social Club is not responsible for the meaning or spelling of symbols or text I provide, and that I am responsible for final spell-checking.

I understand that Heritech Social Club has a NO REFUND policy on tattoos and retail sales.

Deposit Policy

All artists operating within Heritech Social Club are independent artists and are not employees or direct representatives of Heritech Social Club. As independent professionals, each artist establishes and manages their own pricing, scheduling, and deposit requirements.

While deposit amounts are determined individually by each artist, all artists practicing within Heritech Social Club have adopted a non-refundable deposit policy.

Deposits will be forfeited under the following circumstances:

Failure to appear for a scheduled appointment ("no-show").

Cancellation of an appointment within seven (7) business days of the scheduled appointment date.

Requests to reschedule an appointment within seven (7) business days of the scheduled appointment date.

Some artists may choose, at their sole discretion, to allow deposits to be transferred to a future appointment date if the client provides notice of cancellation or rescheduling at least seven (7) business days prior to the scheduled appointment. Deposit transfers are not guaranteed and remain subject to the individual artist's policy and approval.

By scheduling an appointment and submitting a deposit, the client acknowledges and agrees to these terms.
